Liraglutide: where the agencies disagree

This page collects every recorded health technology assessment decision for one molecule across the agencies in our archive, and compares them. The comparison is the point: each agency publishes its own verdict, none publishes how that verdict sits against the others.

What the record shows

NICE (England), SMC (Scotland) and ZIN (Netherlands) recommended or restricted liraglutide, while AWMSG (Wales), CONITEC (Brazil) and TLV (Sweden) did not recommend it.

On the same population the two systems disagree outright: SMC (Scotland) recorded restricted for “For the treatment of adults with type 2 diabetes mellitus to achieve glycaemic control in combination with basal insulin when this, together with diet and exercise, does …”, while AWMSG (Wales) recorded not recommended for “Treatment of adults with type 2 diabetes mellitus to achieve glycaemic control as monotherapy when diet and exercise alo…”.

The file has been open for more than a decade: 16.2 years separate the first record, 23 Jul 2009 at AWMSG (Wales), from the most recent, 16 Sep 2025 at CONITEC (Brazil).

PHARMAC (New Zealand) changed its position: restricted on 17 Jun 2025, not recommended by 27 Aug 2025.

No assessment is on file from CIPM (Spain) and G-BA (Germany), each among the eight highest-volume agencies here.

Not every gap is a refusal: for liraglutide, 1 submission was never made and 3 were withdrawn or terminated before a decision — an outcome set by the company rather than the assessor.

The file is active: 5 of 31 records date from 2023 or later.

Against its own class, liraglutide is an outlier: 56% of its decisive outcomes were positive, where molecules sharing ATC class A10B average 77%.

The programme is broad: 14 distinct indication wordings are recorded for liraglutide by the agencies that label one in English (AWMSG, NICE, PBAC and SMC).

3 of these 31 records carry a year of decision but no day, so any interval quoted here is approximate to within a year.

AWMSG (Wales) was first to assess liraglutide, on 23 Jul 2009; CONITEC (Brazil) did not open a file until 31 Oct 2023 — an unusually long tail of 14.2 years between the earliest and latest system to look at it.

The record arrives in waves: 9 separate years each carry two or more decisions, spread across 11 years in total.

Restriction is doing real work here: 5 of 18 decisive outcomes narrowed liraglutide to a subgroup rather than accepting or refusing it outright, at PHARMAC (New Zealand), SMC (Scotland) and ZIN (Netherlands).

9 of the 9 agencies here assessed liraglutide more than once, HAS (France) most often at 7 entries — repeat review is the norm for this molecule rather than the exception.

9 of the 18 agencies in this archive hold a record for liraglutide.

The full ledger for liraglutide reads 8 not recommended, 5 restricted, 5 recommended and 5 other, with 8 in other categories.
